***Get ahead of the game and get your child's sports physical before the season starts***
Pro Tip: If your child has a doctor's visit or well child check up before July, bring the sports physical form to the visit and have the provider fill out the form. Turn in orginal to onsite registration or at beginning of the season. Physical must have been completed after January 1st of the current year.

- Birth Certificate (certified copy only) or State ID - Physical on the AYF Medical Clearance Form
- Copy of the player's insurance card
- Last report card received
